DR. MINAS CONSTANTINIDES: As we get older, some predictable things happen around the eyes that make them look older or tired. In the upper eyelid, we can get heaviness of the upper eyelid.
Sometimes, very often, that heaviness is from the upper eyebrow coming down, producing that heaviness. But if the brow position is still pretty good, we can actually get marked improvement of the upper eyelid with an upper eyelid lift or upper blepharoplasty.
This operation involves removing a little extra skin that’s heavy or redundant around the upper eyelid and creating a crease of the eyelid it should have been—it should be all along.
That crease often gets lost as we get older. The lower eyelid also gets baggy. Most people notice dark circles or shadows that don’t go away. And a lot of that actually is bagging where the fat that’s naturally there in the eyelid comes forward over time.
So effects of gravity cause the fat to sag forward. The wall that holds it back also gets lax and loose. So just like in the face, the muscle layer gets weak over time. The eyelid wall that holds the fat back gets weak over time.
So we can tighten that wall. And very often these days we don’t remove a lot of fat 'cause that can give an empty hollow look, and we really want to preserve the fullness of the eyelid, which is a more youthful look.
And so we can take that fat and actually transfer it, leave it attached to its blood supply, but transfer it into the hollow areas.
So instead of there being a bulge and then a hollow area where the eyelid meets the cheek and then another bulge where the cheek is, we can make a smoother transition between eyelid and cheek by filling that little indented area with your own fat from the eyelid.
It’s a very natural look. It never gives a pulled look or a hollow look. And it leaves the eyelid looking—giving you back that fullness of youth that many people have lost over time.
